Arletha is a 46-year-old black female with the onset of pain, stiffness, heat, redness, and swelling in both of her knees occurring over the past 5 years. Once in a while she also experiences aching in her hands and she believes that this is associated with weather changes. She has experienced some relief with ibuprofen 600 mg two to three times per day and has basically ignored the problem until today. During her clinic appointment, she has indicated that the pain and stiffness are worse in the morning and when she has been sitting at her desk for a while. She cared for her grandchild over the weekend and indicates that it was getting more difficult to get out of the rocking chair. She denies injury or other initiating events that might explain the signs and symptoms. She does recall her mother having a similar problem. Today she is diagnosed with rheumatoid arthritis.
