The Population Assessment paper is an activity in which the health of a population is assessed, health needs are identified, and primary prevention, secondary prevention, and tertiary prevention services for the population in the student's community are described.
The paper is to be typed in report format. Components of the paper will be in APA format. For readability, there are sections of the paper that can be bulleted or indented lists of data. However, all references need to be cited in the text and a reference page in APA format included.
PURPOSE:
The purpose of the Population Assessment is to demonstrate that you can:
Complete a basic population assessment by accessing necessary secondary data and key informant information.
Summarize the health needs of an identified population in your community
Identify the key problems / need (or key problems / needs) of a populations based on an analysis of the assessment data
Describe primary, secondary, and tertiary prevention resources for the target population in the student's community.
GENERAL GUIDELINES
For the Population Assessment paper, you are to submit a final paper that:
Identifies the population chosen from the list provided
Describes the community in which the population resides (demographic and general community characteristics)
Describes the population's demographic characteristics, health status, risk factors and key informant interview information
Describes at least three health promotion/disease prevention health services for this population in your community
Discusses key informant information about community resources for this population in your community
Provides an analysis of assessment data and community resources including how related to Healthy People 2010 objectives
The Population Assessment paper is due Week 7 . Be sure to use the file naming protocol as the file name and in the header of the document. For example, if Steve Stevenson were submitting the Population Assessment this week, the file name would be, NR478_SteveSt_W7_PopAssess
DATA COLLECTION FOR THIS ASSIGNMENT
The data you will be collecting for this assignment will be a) secondary data sources (existing health statistics data on your population), b) key informant information, and c) professional literature such as journal articles, professional web sites, and your textbook . After you review the grading criteria for this assignment below, review the WEEK 2 RESOURCES for links to accessing data for your Population Assessment, particularly the HELPFUL WEB SITES link. You only need to interview one key informant for this assignment. However, this individual needs to have an understanding of the key health issues and resources for the population in your community. This means you will probably need to interview community agency leaders or directors. For example, if your chosen population is the school-age population in your community, a school health nurse would probably have the best insight into the health issues and community resources. Check with your course faculty if you are unsure as to the type of key informant to interview for this assignment.

Who we are...


Who We Are
Initially, this ministry began as a vision to feed the hungry, poor, needy and homeless of Santa Ana. The founder, through experiences gathered as a volunteer in other ministries, brought together a small group of people to fulfill this vision. This small group consisted of members of a local Hispanic church, along with other community members.

In early 2008, we arranged to pack grocery bags of food at an off-site warehouse and transported the ready bags of food to our parking lot in Santa Ana. Flyers were distributed to the nearby residents and the first 50 grocery bags were passed out. This process continued every first and third Saturday of each month. Bags of food were also delivered to the homeless population. The word spread and after several months, we purchased and delivered pallets of food and began packing the food on site during the distribution event. Tents, tables and chairs were purchased to accommodate the recipients. Soon, an 18 foot bed truck, fork lift and pallet jacks were purchased to facilitate the distribution. The excess non-perishable food was stored in our secured subterranean area.
After realizing the community's other needs, clothing, shoes, diapers, baby food, school supplies, toiletries and other small household products were quickly added to our distributions.
In 2009, the demand grew and two nearby warehouses totaling 6400 sq. ft. were rented. On June 9, 2009 , we obtained our 501(c)(3), non-profit status and opened our free medical clinic operating two days a week , providing general medical care for the entire family.
In 2010, a third warehouse totaling 4800 Sq. Ft. was rented and we began serving the people with Free Legal Aid and an Advocacy Service to help prevent people from losing their homes.
In the future, we hope to expand the days of operation and add additional specialists and services to the free medical clinic, free legal aid, food pantry and advocacy (housing) programs. We continue to work closely with the community to determine other basic needs that are currently unavailable or unaffordable to them and thereby add such services to our organization to Serve The People.
