Essay Instructions: The purpose of this assignment is to provide the learner an opportunity to design a health promotion proposal specific to your role specialization.
Each student will write a scholarly paper that demonstrates graduate school level writing and critical analysis of existing nursing knowledge. The paper will be a minimum of 12 pages and maximum of 15 pages, using APA style.
Choosing Your Topic
The health promotion project is on chronic kidney patients aged 40 to 60 years of age. Theoretical frame work will be Nola Pender?s Health Promotion Model. The perspective it should be written as is that I will be a Nurse Practitioner and how I as a Nurse Practitioner will execute this Health Promotion Proposal regarding Chronic Kidney disease and evaluate it. Evaluation will be measured by a questionnaire.

Essay Instructions: The health promotion goal I propose to address is the reduction of the United States (US) population with chronic kidney disease (CKD).

The project needs to have a goal that is clearly articulated and relates to the role of the Advanced Practice Nurses (APN).

Case Study
Ali is 67 years old and has chronic kidney disease. He worked as a manual labourer, often smoking
during outdoor work, as he mainly worked alone. In addition, Ali drinks heavily and consumes at least
6 cans of beer each night. He was diagnosed with hypertension 15 years ago and his medication
dosage has increased over that time. More recently, he has been short of breath, has periods of nausea
and vomiting, and has been generally tired. At a check-up with his renal specialist, it is found that Ali
has large concentrations of protein in his urine, his creatinine level is 497 mmol/L and his estimated
GFR is only 11 mL/min. The specialist recommends that Ali commences dialysis.
