Essay Instructions: This paper is a capstone project for an acute care facility. The proposed practicum project is FALL PREVENTION: Working with your Care Team.
Enclosed are the specific instructions and followed by the proposal and objectives to set forth. This paper must be written in the third person. Each objective must have a methodology, formative eval, summative eval. The paper must contain 5 literature reviewed references. Please use QSEN INSTITUTE OF MEDICINE REPORT. Please add in a table or graph. There are three objectives and each one must have its own methodology.
The practicum project plan is written as a scholarly proposal for your project. Imagine you are submitting this proposal to your leadership team for review and approval. Consider what information you would want in a project proposal if you were an executive who was responsible for the use of human, financial, time, and other resources. (Note: For this assignment, you do not have to address the financial aspects of your plan.

Title. Include the name of your project in the title, following the Uniform-Guidelines format. FALL PREVENTION PROGRAM: Working with your care team. This is a pilot project proposal to assist one unit in a hospital who has had the most patient falls and one with a serious injury. This project is to prevent falls and maintain patient safety.
Introduction. In the introduction, include the purpose of the paper, the goal of the project, the name of the project, and the structure of the paper.
Goal statement. A goal statement identifies what you expect to accomplish, a focus area, and the population. The goal statement introduces the project and relates, in broad terms, how you will solve a particular problem or issue. For example: To develop a curriculum on patient safety that is needed to meet state guidelines.
Project objectives. Project objectives delineate your strategy for reaching the goal. Remember that project objectives are different from your practicum-experience objectives. Project objectives are the steps you will follow to complete the project. You must include a minimum of three objectives, using Bloom’s Taxonomy as a guide in selecting the action verb for each objective. Select verbs from the Application level of this taxonomy and higher.
Evidence-based review of the literature. This review must include a review of your specialization’s professional-practice standards and guidelines related to your project. Evidence-based support for your identified problem, project methods and evaluation are expected. You must include a minimum of 5 scholarly references.
Methodology. The methodology section addresses in detail how you will accomplish the objectives. Include, as relevant, the "who," "when," "where," and "how" of each objective.
Resources. Identify and justify the human, physical, and/or technical resources you will need to complete this project.
Formative evaluation. Describe how and when you will conduct formative evaluation of your project. Relate how you will use the evaluation results. Do not stop with a definition of formative evaluation. Consider how you will know the project is proceeding as planned.
Summative evaluation. Describe how and when you will conduct summative evaluation of your project and how you will use the evaluation results.
Timeline. Create a graphic timeline representing significant stages of your project. Supply a narrative sufficient for your Instructor to understand the timeline. Place the timeline in an Appendix to your project plan.

Fall Prevention Program: Working with your Care Team
Falls are the leading cause of hospital related injuries in the United States. They involve patients of all ages. Inpatient falls are serious patient safety and quality issues. Patients injured in a fall incur increased hospital costs due to additional treatment and longer lengths of stay (Pearson, Coburn, 2011). Fall prevention is a critical component of any patient safety approach. Effective communication among staff, patients, and their families augment information transmission, build relationships, and increase aptitude for positive patient safety and culture change. According to the IOM, (Institute of Medicine) creating safe, effective, patient-centric, timely, efficient, and equitable care has become the structure for the national patient safety movement, built upon clinically excellent care (Joint Commission National Patient Safety Goal, 2014).
Project Objectives
• Implement a pilot program with a purpose to reduce the number of patient falls and preserve patient safety.
• Contribute to the practicum project by coaching and mentoring the staff to be decision makers and teaming with the patients to prevent falls.
• Analyze the results of the pilot project related to the purpose and objectives.
Evidence
Quite little is known about the extent to which fall prevention interventions can be successfully implemented in an acute care facility. Systematic reviews and meta-analyses of falls rely on randomized controlled trials, which are challenging to perform. Evidenced-based literature on falls and fall prevention focuses more on community settings rather than hospitals (Rand, 2003). The research literature review used to assist in finding the best evidence based process would be to use the (PICO) method, the acronym for Population, Intervention, Comparison and Outcomes.
Prior to implementing this project we will have scheduled focus groups to collaborate new ideas and strategies to make this project work. Coaching and mentoring the staff to move forward and to delegate to the nursing assistants and to get all team members on board.
Formative Evaluation
During the formative evaluation, my preceptor and I will re-group with the team and discuss the progress of my project. Time is a major factor for this particular project due to limited practicum hour requirements (Davey, 2007). This is the time to focus on positive results and build toward the summative evaluation.
Summative Evaluation
By the end of the practicum project, the learner and the preceptor will review the efficacy of the fall prevention program: Working with your Care Team; and the maintenance of patient safety. We will compare the total number of patients on the program to the number of falls during the six week trial period.
