CASE STUDY

1000hrs Mr White, a 54 year-old bank chief executive officer is brought in by ambulance having

complained of severe crushing pain in the centre of his chest. He feels faint and sick. The pain began

30 minutes earlier.

History obtained from wife

Mr White was diagnosed with hypertension in 2008 and was prescribed enalapril. One year ago he

was diagnosed with high cholesterol and prescribed Simvastatin.

He has smoked 20 cigarettes/day since the age of 15 years and drinks a few beers at night.

Mr White?s father died of a heart attack at age 45, his mother had a stroke at 77 and resides in a care

facility.





There is no history of serious illness or allergies.

Vital signs

Blood pressure 95/60

Pulse rate 100bpm and regular

Respiratory rate 18/min

Temperature 37.10C

Laboratory tests

Full blood count ? all values within normal range

Creatine kinase 100 IU/L

Troponin T 0.01 ug/L

Blood glucose: 6.5mmol/L

Electrolytes, urea and creatinine ? all values with in normal range

ECG - frequent PVCs and ST segment elevation

Diagnosis

An anterior wall, transmural, ST elevation myocardial infarction affecting the left anterior descending

artery. He is assessed as being eligible for thrombolytic therapy

Management

Oxygen therapy via nasal prongs at 4 litres/minute.

Sublingual glyceryl trinitrate, without improvement in his pain

Aspirin 300mg orally stat

IV bolus dose of 2.5mg of morphine in 2.5mL of N/S to relieve pain, followed by an IV bolus dose of

10mg Metoclopramide?.

He is given a bolus of heparin followed by two bolus doses of Alteplase?

He is then started on a heparin infusion and his APPT is maintained at 60 ? 80 sec. He is also given

an intravenous infusion of glyceryl trinitrate to relieve his angina pain and an injection of frusemide to

reduce his cardiac failure. He is transferred to the Coronary Care Unit (CCU) for monitoring.
