Another statement regarding the health problem of a patient is the possible diagnosis which tends to declare about a problem that the patient most probably has. Although due to lack of information an accurate diagnosis is not possible. Further on there is an actual diagnosis is the diagnosis of a health problem that the patient has and nursing care can be beneficial for the patient. Moreover a syndrome diagnosis and a wellness diagnosis is when multiple nursing diagnoses are observed and to describe the characteristic of the patient when at a high state of wellness respectively (Wikipedia).

Collaborative problem on the other hand "is an actual or potential health problem (complication) that focuses upon the pathophysiologic response of the body (to trauma, disease, diagnostic studies, or treatment modalities), and that nurses are responsible and accountable to identify and treat in collaboration with the physician" (Anonymous).

Hence it should be noted that collaborative problem is where the identification and treatment is not done by nurses independently and is a collaborative effort whereas nursing diagnosis is where the nurses identify and treat the health problem independently without any collaboration.

Potential Barriers to Successful Collaboration

The potential barriers that would hinder the process of collaboration between different groups in the health care sector would be any lack of a clear vision or leadership. Communication gap would prove further hindrance in the process. Like seen in the example above regarding the Katrina Tragedy, there was an efficient system of communication and coordination. Poor data compilation or storage as well as sources hard to amalgamate would prove to drag the collaboration down. Information and records should be computerized and hence if the health professionals are not computer literate in terms of sharing, adding and generating computerized data then that would surely prove to be a problem. Any lapse in these would cause a complete disaster in the collaboration. Moreover if the population that the collaborated care is to be delivered is small, capitation can be a real problem. At yet another level, lack of trust, respect as well as organizational factors along with lack of common language can be a potential barrier to successful collaboration.
Adequate funding is also important as well as their allocation to services and providers. If the funding is not sufficient then it would make it harder for effective collaborative care to be implemented. Changes in practice patterns along with proper labor substitution are important to achieve the desired goal.

CONCLUSION

Collaborative care has in times of need proven itself to be really effective. A network of different healthcare units and service providers come in contact to other sectors and units to form a colossal effort in serving the patients even in times of state emergency like seen in the Katrina tragedy case. Although there are barriers and factors that might lead to be barriers later on, the community and society should work together to overcome such lapses and to promote the acceptance and practice of collaborative care.
